Health Information

Overview of Ohio Immunization Summary for School Attendance:

• KG students must submit immunization records.
• 7^th Graders are required to have the DTap/DT Tdap/Td (Diphtheria, Tetanus, Pertussis) vaccination and first dose of Meningococcal (MCV4) vaccination.
• 12^th Graders must have the second dose of the Meningococcal (MCV4) vaccination

Click here for the immunizations that are required unless exempted for medical or religious reasons. Ohio Revised Code, Section 3701.13 and 3313.671, mandates that every child be properly immunized in order to attend school. Exemption forms must be submitted each school year. Ohio State law allows a 14-school day period for you to provide a record of your child's immunizations before being excluded from school. 

Where to Send Vaccination Records:

After your student has been vaccinated, please send records to their respective building nurse. You may email records or have your student’s physician fax records directly to the building. Contact information for each building is below:

Paying School Fees 

With PaySchools Central, you are able to manage your student’s accounts & fees in one platform, even allowing you to set up an auto-replenish option to automatically refill lunch accounts when they are running low.

Simple Set Up Steps

1. Create your account using your computer or tablet by visiting: www.payschoolscentral.com 
2. For mobile-friendly use, download the PaySchools Central app from the Apple Store or Google Play.
3. Complete account registration by selecting register, entering your profile details, setting up your password via the email link that will be sent to you, then logging in.
4. Add your students to your account using their student ID numbers.
5. If desired, enter credit card and/or banking information as forms of payment to use with your PaySchools account, or simply plan to use guest checkout when prompted.
