Emergency Medical Authorization Forms

All students must have an EMA form on file in the school office. This form provides the school with several emergency telephone numbers for family members as well as family doctors and dentists. A student without an EMA on file will be prohibited from taking part in field trips or athletic events. Forms are given out to students the first day of classes and extra forms are available in the school office. Please keep the office informed of new emergency/home telephone numbers.

Medication at School

If any student needs to take doctor prescribed medication or non-prescription medication at school, the following procedures must be followed:
    1) The proper form must be completed by a physician and parent/guardian and returned to the school office:
      a) Parental/Guardian Authorization & Release Section
      b) Physician's Request for the Administration of Prescription/Non Prescription Medication by School Personnel
      Both Sections of this form must be completed.
    2) Parents or guardians must bring, in person, the prescription/non prescription drug to the school office.
    3) The drug must be in the original container in which it was dispensed by the physician or pharmacy. The container must be marked with the drug name, dosage, doctor's name and patient's name.
We suggest that parents keep these forms and take them along to the doctor/dentist if they anticipate the need for their child to receive medication for an illness. Additional copies of these forms are available in the office.
