Participation Forms
The following is a list of the forms that will need to be completed for your student athlete.
- Pre-participation Physical Evaluation must be completed by a physician and will be valid for one calendar year. If it expires mid-season, the athlete must turn in another physical before they will be allowed to participate. The best time for a physical is the summer months to avoid any participation problems. This form is required prior to participation.
- OHSAA Authorization Form: Medical release content, eligibility, student responsibility form. This form is also valid for one calendar year and is required prior to participation.
- Emergency Medical Authorization Form: to be filled out with each season and kept with the coach. A new form must be given to the coach with each new season and is required prior to participation.
- Handbook / Student Participation Form: Handbooks are given to every athlete once per season. The back page will need to be filled out and turned into the coach after team selections.
- Pay to Play fee of sixty five dollars is due for every sport that a student athlete participates in and must be paid prior to the first competition.
Academic Eligibility
Academic Eligibility requirements must be made to compete in interscholastic athletics; all students are required to meet the following three-fold standard:
All LMJH student-athletes must meet the O.H.S.A.A. academic requirement which states: A student in grade 7 or 8 must be currently enrolled and must have been enrolled in school the immediately preceding grading period and received passing grades during that grading period in 75% of those subjects in which the student receives grades.
Consequence: If the student fails to meet the O.H.S.A.A. academic requirement of passing 75% of the subjects in which they were enrolled, the student will be ruled ineligible to compete in interscholastic athletics for the duration of the next grading period. (O.H.S.A.A. Policy)
By school board policy, student athletes attending Little Miami High School must achieve a 1.0 grade point average in course work taken the preceding interim grading period in order to maintain interscholastic athletic eligibility in good standing. Grade point averages will be checked weekly with one wavier per year. Eligibility for play will be determined on a weekly basis as follows:
A student with two (2) Fs is ineligible.
A student with one (1) F and two (2) Ds is ineligible.
A student with (4) Ds is ineligible.
Ineligible students must attend Study table.
All fall athletes will qualify academically by their grade performance in the last 9 week grading period of the preceding school year. Athletes cannot re-establish athletic eligibility by taking coursework in summer school.
